NAPO (Top Section)

  1. Only one copy is necessary (2 copies for CAS).
  2. Follow regular NAPO instructions. If this is a new position, other than a research or instructional position, please enclose a statement from the Office of Human Resources confirming that the position is cleared for unclassified status.
  3. Route form for “Review and Approval” signatures. Office of VP for Academic Affairs will return a final copy with all signatures to the “Contact Person” shown at the top of the form.


CAMPUS VISIT REQUEST (Bottom Section)

  1. Make a copy that includes all the NAPO “Review and Approval” signatures. Fill in campus visit information by hand.
  2. Collect copies of candidate’s credentials to be included with form (see instructions below).
    Required:
    a. Curriculum vitae or resume
    If available:
    b. Letters of recommendation (three)
    c. Other supporting material
    d. Publications
  3. For fixed-term and tenure-related faculty candidates, submit two copies of this form (three for CAS) with one set of material listed in Item 2 above (two for CAS) to the Dean’s Office two weeks before the proposed visit.
  4. For a position involving indefinite tenure, include three sets of material listed in Item 2 (three for CAS).
  5. Upon approval, a copy of the form will be returned to the department.
