SABBATICAL LEAVE INSTRUCTIONS
Complete the application form in triplicate (except CAS, COE, and AAA
need four copies). The signatures on all copies must be original.
At least one copy of the application form
must be two-sided, with the Oregon Administrative Rules printed on the
back. Complete
the entire form, including academic ranks with dates of service and
previous sabbatical leave dates.
Attach supporting documentation:
a. Complete statement of the purpose of the sabbatical leave
b. Current curriculum vitae
c. Description of current teaching, research, and other professionally
relevant activities
Proper routing in sequential order:
1. Department Head approval
2. Dean/Director approval
3. Vice Provost for Academic Affairs for approval and verification of
eligibility, service and salary. If the faculty member does not qualify
based on the accumulated FTE but qualifies based on length of service,
the Vice Provost for Academic Affairs may negotiate an appropriate leave,
with the approval of the Dean.
4. President (or designee) for final approval.
5. Resource Management for final distribution
Distribution:
1. Original into faculty member’s academic file (ORM)
2. Original or copy to Dean/Director
3. Original to appropriate Vice President
4. Original to department
Payroll processing:
Departments are responsible for preparing separate paperwork to adjust
pay for an approved sabbatical leave. Complete
a Payroll Request Form (PRF) and route
through normal channels. Use account code 10123 (Sabbatical Leave). To verify
that the paperwork was completed correctly, the department should check HRIS
(PWIVERI) for adjusted pay and correct FTE.
