Resignations, Terminations and Retirements

For all resignations and terminations, submit as a packet the following documents through the Dean’s/Director’s office to the appropriate vice president's office. The vice president's office will forward the packet to the Payroll Office and the Office of Resource Management for the faculty file.

1. Payroll Request Form (PRF) to terminate the job and, if appropriate, pay off unused vacation.
2. A signed letter of resignation. If this is not possible, an explanation is required.

The vice president will send a formal acceptance and acknowledgement of resignation to the resigning employee (copying the offices of Employee Benefits and Payroll).

For all retirements, submit an Unpaid Appointment Form (UAF) using the position number BUOXRT to set the employee up to receive post-retirement services. Submit the form through the Dean’s/Director’s office to the office of the appropriate vice president. The vice president's office will forward the UAF to the Payroll Office and the Office of Resource Management for the faculty personnel file.
